Pistol malfunctions: read the symptom, then look for the cause

Verified by a gunsmithA pistol that jams does not say why. It shows a symptom — the slide will not close, a case stands proud, the round does not fire, two cartridges present at once. Here is how to read each symptom, which drill to run immediately, and which causes to look for after, ordered by real frequency.

The immediate drill, before any diagnosis
This is the separation almost every page on the subject misses: the drill and the diagnosis are not the same moment. The diagnosis can wait, safety cannot.
Whatever the malfunction, three things come first:
- The pistol stays pointed at the target. A malfunction is not a reason to turn the muzzle towards yourself for a look.
- The finger comes off the trigger. A round that has not fired can still fire on the next movement.
- You do not lean over the ejection port. A cartridge that ignites during handling throws the case outwards: Smith & Wesson makes this an explicit warning, keep your face away from the ejection port4.
Only then comes the diagnosis. And there, the symptom you observe points almost always to the right family of causes.
The round does not fire: the one dangerous case
The other malfunctions immobilise the pistol. This one can still make it fire.
A failure to fire can be a clean dud — the primer did not take — or a hangfire: the primer took, but ignition is delayed. Nothing separates the two at the moment the round fails to go off. That is why the first rule is to handle nothing, and to wait.
For how long? This is where the subject becomes genuinely badly documented. We opened the manufacturers' manuals one by one. They all publish a figure, and they do not publish the same one.
Why the manuals, and not a federation. We went looking for a governing-body rule first. The French federation, the FFTir, publishes safety rules but nothing on failures to fire, hangfires or clearing a stoppage — verified on its own safety page5. That absence is what sent us to the manufacturers. If your own federation publishes a figure, it governs over anything below.
Hangfire: how long to wait, according to the manufacturers

The four values come from the manufacturers' own manuals, opened one by one1234. They do not contradict each other on the principle — keep the pistol pointed at the target and handle nothing — but they do on the duration, by a factor of six. With no common rule, taking the longest costs nothing.

Walther prescribes ten seconds1, CZ thirty — noting that a hangfire "will fire within 30 seconds"2 — Beretta "one (1) full minute"3, Smith & Wesson sixty seconds4. A factor of six between the shortest and the longest, on the one malfunction that can injure.
They agree on everything else: pistol pointed at the target, no handling, then unload, then a visual check of the bore. That last step is not a precaution of principle. If the round went off weakly instead of not at all, the bullet may have stayed in the barrel — Beretta names this case and warns that it produces a dangerous barrel obstruction3. Firing behind a lodged bullet is the most common way to destroy a pistol, and sometimes a hand.
With no common rule, take the longest. Waiting sixty seconds rather than ten costs nothing on the range and covers all four manuals. And if your pistol is not on this list, its manual says: that is what governs, not this article.
Two cartridges present at once: the double feed
One cartridge is already in the chamber, a second rides up behind it, and the two jam. The slide no longer closes and the pistol is frozen.
It is the longest to clear, for a simple mechanical reason: as long as the magazine is in place, it keeps pushing. Working the slide does nothing. The magazine comes out first, and only then does the chamber free up.
It is also the malfunction that points most directly at its culprit. When experienced shooters explain a double feed, they come back almost always to the magazine — spread lips, a tired spring, a follower that catches.

The case stands proud: the stovepipe
The case was not fully ejected: the slide closes on it and pinches it, vertically or at an angle. Visually spectacular, mechanically benign.
The cause is almost always sluggish ejection — the case was extracted, but without the energy to clear. Three leads, in this order: underpowered ammunition, a fouled extractor or port, a recoil spring at the end of its life. The slide will not close, or stays locked
Two situations often confused, though they share neither cause nor seriousness.

That third line deserves a word, because it is regularly taken for a breakdown when it is a matter of grip: the shooter's thumb touches the slide stop while firing. The pistol is sound; it is the hand position that changes.
The causes, ordered by frequency
The natural reflex is to suspect the pistol. That is almost always the wrong order.

Break-in deserves a nuance. Several manufacturers note that parts only move freely after a few hundred rounds: an isolated malfunction on a new pistol therefore does not carry the weight of one on a run-in gun. What decides is not the malfunction, it is the trend — a falling rate is a break-in, a steady rate is a defect.
Which still means being able to measure it. That is the whole point of tracking your malfunctions: three columns — pistol, ammunition lot, magazine number — are enough to turn an impression into an attribution.
An isolated malfunction proves nothing. Stoppages happen to every pistol and every shooter. What deserves attention is a repetition on the same pistol, the same lot or the same magazine — and that is precisely what cannot be reconstructed from memory at the end of a season.
